What To Expect

Please arrive promptly for your scheduled appointment. If you are unsure as to your scheduled time
please call our office at (520) 322-5012.  For directions visit our Contact page.

If you use hearing aids or glasses, please be sure to bring them with you to your
appointment. Casual, comfortable attire is acceptable.

Testing may last until 3:00pm with a lunch break.  There is a refrigerator and microwave available if you prefer to bring your lunch.  There is a soda machine in the building and fast food restaurants are near by. We will provide coffee and tea.

For your convenience you may download and print out the necessary forms.  Please fill out the forms in their entirety and bring them with you for your scheduled appointment.  If you are unable to print out the forms, you may fill them out when you arrive.
